What are your hours of operation?
We are open Monday through Thursday 8am to 6:30pm, Fridays 8-3pm, and Saturdays – by appointment only.
How long is each session?
All sessions with our therapists are scheduled for one hour.
What do you need for your first visit?
SDK Therapy requires a prescription for OT or PT from your referring provider to treat the issue you would like to be seen for. Please have the referring provider's office fax (212-421-1750) or email (info@sdktherapy.com) the script in advance of your visit, or bring a copy to our office.
We also recommend that you complete our electronic intake form, prior to your first visit.
What to expect during your initial evaluation?
Please arrive 15 minutes early to ensure all paperwork is complete.
During your evaluation we will learn more about you - your relevant medical and social history, how the issue/condition is impacting your daily activities, and the goals you want to achieve. We will take measurements, and help answer any questions. At the end of our evaluation, we will assess your condition, suggest a treatment plan, including patient education material, and prepare you for treatment at your next visit.
What is your cancellation policy?
We designate an hour for all of our patients. We value your time and our time – if you cannot make your appointment, please call us 24 hours in advance to avoid a $50 cancellation fee.

Feel free to call our front desk with any additional questions. If you have clinical questions, one of our therapists can give you a brief call prior to scheduling, and answer any additional questions.
